Shift: 8 Hours - Day Shifts

Job Schedule: Full time

Weekly Hours: 40.00

Starting Rate: $17.00

Job Summary

This position requires a detail orientated person. Must be able to interact with patients and/or families with good listening skills and compassion for their request of hospice services. Responsible for the processing of new patient/client requests and tracking admission status. Coordinates admissions process, completes and scans paperwork into Health Information Management (HIM) system and orients patients/clients to location. Processes HIM paperwork throughout the patients stay on service. Ability to deliver high quality customer service in a professional manner. Strong interpersonal and listening skills.

Ability to handle confidential and sensitive information.

Must be able to coordinate several different responsibilities at once.

Acts as a support person to facility by completing administrative tasks such as answering phones, taking messages and answering basic location questions.

Coordinates patients’ admission/intake process, with client and location employees in compliance with state and federal regulations.

Ensures smooth transition from referring facility to location.

Communicates with discharge planners, family members and members of the interdisciplinary team to promote the location and the use of services provided.

Enters data in to the electronic medical record (EMR) accurately.

Completes medical records from admission to end of stay; includes scanning and indexing into HIM application.

Will be responsible for the admissions and coordination of services for clients.

Sets up transportation from patient location to and from Inpatient Unit when services are needed for General Inpatient Care or Respite.

Orders Medical Equipment (DME) upon admission for home and inpatient unit admissions.

Handles pre-billing process for claims.

Knowledge of health insurance applicable to a hospice setting.

Orders supplies as needed.

Qualifications

High school graduate or general education diploma (GED) is preferred.

Prior work experience in long term care facility preferred.

Based on facility needs, may require a current valid driver’s license, and must meet all medical guidelines for Sanford Health Category III drivers.
