Job Description Job Description RN Case manager Job Summary: The
RN Case Manager plans, organizes, and directs home care services ,
ensuring compliance with CMS guidelines . This role involves
coordinating patient care , developing individualized care plans ,
and collaborating with interdisciplinary teams to optimize patient
outcomes. Key Responsibilities Complete initial assessments of
patients and families to determine home care needs . Develop and
revise care plans based on patient status. Perform preventive,
rehabilitative, and therapeutic nursing procedures . Counsel and
educate patients and families on health care needs. Qualifications
Current Registered Nurse (RN) license in the state of practice
(Required). CPR certification (Required). One year of home health
care experience (Required). Experience in wound care, vacs care,
ostomy care, and IV administration (Preferred). Skills & Abilities
Strong knowledge of OASIS-E documentation . Ability to exercise
