Job Summary

The Licensed Master Social Worker supports the vision of the Total Healing Environment and the values of respect, integrity, accountability, commitment and quality while providing timely and accurate identification of patients' psychosocial needs along with the development and implementation of plans of care for post hospitalization. Works collaboratively with the multi disciplinary team to provide input and assistance assessing, planning, evaluating and implementing care across the Continuum. Plays a key role in oversight of discharge plans with those individuals needing intensive psychosocial needs.

Job Qualifications

Required Qualifications:

* Master's Degree in Social Work (MSW) from an institution accredited by the Council on Social Work Education or an equivalent foreign degree as determined by the Foreign Equivalency Determination Service of the Council on Social Work Education or Master's degree in a complementary field such as psychology or counseling, with licensure in the State of Arizona from the Board of Behavioral Health
* Current State of Arizona Licensed Clinical Social Worker (LCSW), Licensed Master in Social Work (LMSW) by the Arizona Board of Behavioral Health Examiners, Licensed Professional Counselor (LPC) or Licensed Associate Counselor (LAC)
* Three years post graduate experience as a social worker or complementary professional experience in a hospital, other medical or psychiatric environment
* Excellent interpersonal skills and the ability to effectively communicate verbally and in writing providing excellent customer service
* Basic computer literacy and proficiency in Microsoft Windows
* Basic proficiency with MS Office (Outlook, Word, Excel, PowerPoint, Publisher)

Preferred Qualifications:

* Electronic Medical Record (EMR) experience preferred; Cerner strongly preferred
* Must pass pre-employment screening
