Roles & Responsibilities Assess and manage patients in the ED using InterQual Criteria to determine appropriate level of care Coordinate care across the continuum by collaborating with physicians, social work, admissions, and community resources Facilitate safe and efficient patient throughput, including admission status recommendations and outpatient disposition planning Provide targeted care coordination for patients with mental health needs, frequent utilization, chronic pain, and abuse Develop and participate in creation of clinical practice guidelines and physician order sets to improve quality and reduce costs Lead discharge planning and arrange referrals to all levels of care to support safe transitions Document interventions and care plans accurately in the EMR and communicate effectively with multidisciplinary teams Qualifications Active, current, full and unrestricted RN license in any US state, territory, or Puerto Rico Associate Degree in Nursing (required) Basic Life Support (BLS) certification by the American Heart Association (current) Obtain Case Manager Certification (CCM or ACM) within 2 years if not already certified Minimum 2 years of current care/case management experience in the Emergency Department Minimum 2 years patient care experience (hospital, home health, or skilled nursing) At least 1 year of travel nursing experience Fingerprinting completed and cleared prior to start Education Associate Degree in Nursing Knowledge & Skills Proficient with Electronic Medical Records and documentation best practices Comfortable using Microsoft Outlook, Word, and Excel for communication and reporting Skilled in patient assessment, care coordination, and discharge planning Experience applying InterQual Criteria for patient status determination Strong interdisciplinary collaboration and resource management capabilities Ability to work effectively with patients across the lifespan, including those with mental health needs Familiarity with developing clinical practice guidelines and physician order set input Benefits Shared contractor housing available (hospital housing is limited) — a simple, ready-to-go option while you settle in Supportive interdisciplinary team that values your clinical judgment and mental health expertise Assistance with fingerprinting and credentialing to get you started smoothly Licensure guidance and support — practice with an active, unrestricted RN license from any US state/territory Opportunities to shape clinical practice guidelines and have a visible impact on patient care Professional growth—complete Case Manager Certification (CCM or ACM) within two years with employer support Meaningful patient-focused work serving a culturally rich community on the Navajo and Hopi lands Work-life balance with scheduled weekend rotation and defined on-call expectations Travel Perks: Explore Tuba City, AZ Tuba City sits on the Colorado Plateau and offers a culturally rich experience near Navajo and Hopi communities.
