Hiring Bonus $5000.00Assesses and formulates a plan of care for clients in their homes that includes social, emotional, and physical needs.Provides nursing care to clients according to physician orders and regulations.Provides teaching and training to client/family/caregiveras appropriate.Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in the specific care area.Communication skills to collaborate, negotiate and resolve conflicts, situations, and/or issues with peers, inter-professional colleagues, patients and families.Must have strong clinical reasoning and critical thinking skills to analyze data and appropriately respond to changing situations, coordination of care, patient assessment, patient education, triage, and various other nursing interventions.Adjusts departmental priorities and resource allocations to support changing needs.Applies general clinical skills to perform clinical functions or tasks.Applies home healthcare operating principles and practices within own position.Educates and trains others on operations, ethics, and regulations within the home healthcare industry.Effectively delegates and supervises based on one s state practice act and an understanding of the concepts of responsibility, authority, and accountability.Establishes and maintains productive working relationships within and outside of own area.Fosters strong customer relationships via delivery on commitments, open communication, and on-going feedback/improvement.Promotes an organizational culture that balances individual initiative and team collaboration and sharing of successes.Reinforces positive contributions and recommends adjustments for improving outcomes.Responsible for promoting a safe environment for patients, families, staff, residents, and visitors and working collaboratively with the multi-disciplinary health care team to provide the best outcomes for patients.Seeks opportunities to serve as a mentor.Uses active listening skills and probing techniques to understand cause of a conflict.On Call RotationQualificationsBachelor's degree in nursing strongly preferred. Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).Minimum of two years home health/hospice nursing experience preferred.Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications. Depending on location, a valid drivers license may be required. Basic Life Support (BLS) required.
