Job Description: Job SummaryThe RN Care Manager supports the vision of the Total Healing Environment and the values of respect, integrity, accountability, commitment and quality, facilitating appropriate admissions, transfers, assessments and discharges with alternative placement in a manner which effectively and efficiently provides for patient care needs; seeks to ensure the medical plan is implemented in a quality, cost- effective manner; encourages the involvement of the patient in the decision making process to obtain optimal health and wellness and a multidisciplinary approach to the plan of care and services across the continuum.Job QualificationsRequired Qualifications:* Must hold a current, valid Arizona or compact state RN license* One year of care management experience in a hospital setting; may consider three years of acute care medical/surgical or population health management RN experience as equivalent experience. Three years of hospital based care management experience preferred.* Knowledge of admission and continued stay criteria* Knowledge and skills necessary to evaluate appropriate care for patients from neonates to geriatrics* Knowledge of federal, state and managed care rules and regulations including CMS and AHCCCS* Excellent interpersonal skills and the ability to effectively communicate verbally and in writing providing excellent customer service* Computer literacy and proficiency in Microsoft Windows* Basic proficiency with MS Office (Outlook, Word, Excel)Preferred Qualifications:* Bachelor's degree in Nursing* CCM certification* Experience with INTERQUAL or Milliman* Electronic Medical Record (EMR) experience; Cerner strongly preferred* Must pass pre-employment screeningCompany OverviewYavapai Regional Medical Center is a not-for-profit, integrated healthcare provider that offers a broad range of inpatient and outpatient services.
